Living with a Cardiac Defibrillator by Mike J King
The last fifteen years has seen a massive increase in arrhythmia related medical conditions. Suddenly people in the street have actually heard of Sudden Adult Death Syndrome (SADS).
Much of this increase in such conditions is undoubtedly the result of the increasing stress placed upon each of us as we pedal ever faster on the hamster's wheel. How do we solve this particular issue? One friend of mine suggested that we deport all management consultants but I think this was a joke. Funny, isn't it, by the way, how the management consultant has become the twenty-first century pariah. The once despised auditor is now welcomed with open arms (he, of course, has the professional indemnity insurance).
Levity aside, in this article I am going to talk about the second reason for this massive increase. That is, technological advancement and the increasingly sophisticated therapies offered to sufferers of conditions such as Hypertrophic Cardiomyopathy, Long QT Syndrome, Short QT Syndrome, Arrhythmogenic Right Ventricular Cardiomyopathy amongst many others. In particular, I am going to discuss the internal defibrillator, from the inside, as it were.
Sixteen years ago when I received my own internal defibrillator most people had no idea that it was possible to implant those 'paddle things' (as seen on Casualty et al) into the abdomen of patients. (It is a sign of how long I have had my own particular lodger that I should say abdomen - all defibrillators are now implanted in the shoulder area). Whenever I was rushed to the local hospital with some kind of arrhythmia related emergency, the word went out and doctors flocked to see the patient with the defibrillator (that was me, by the way). My X-rays were pored over as if they held the answer to the DaVinci code (of course, it wasn't written at that time but that wouldn't stop Dan Brown, would it?). How things have changed now that thousands of people have received these little friends.
So, what is it like to have an internal defibrillator? I suspect that if you asked all defibrillator patients this question you would get as many different answers. The truth is that one's opinion is essentially linked to two things.
The first is the extent to which one has adapted to one's new status in society. Immediately prior to my cardiac arrest and first defibrillator I was the Finance Director of a subsidiary of a PLC listed company, (I considered myself to be very important), earning about £35k to £40k per year (and with prospects). Now I 'earn' nothing and receive £70 per week incapacity benefit and have no 'prospects', (I am currently waiting for Gordon Brown's knock on the door and my re-integration into the work ethic). There is a lot of opportunity there for bitterness. However, fortunately my priorities have changed and I now thank my lucky stars that I can be an integral part of my children's growing up. Money has ceased to be an issue for me (well, it is in so far as I have none but as long as we can get by I don't mind). On this basis, I do not resent the thing in my 'stomach'. (Are there subconscious shades of 'alien' in the way I put that?).
The second is one's direct experience of the defibrillator in action. For many years I found it difficult to be anything other than afraid of it. And to be honest, I had good reason. I have had countless shocks from it, including in 1993 when I suffered forty consecutive shocks that eventually led to my heart stopping for three to four minutes (no out of body experience, unfortunately). Interestingly, the defibrillator recorded this as a successful therapy (my heart stopping also stopped the ventricular tachycardia - sort of the same philosophy of the current Government, as it happens - 'the reality is unimportant as long as the statistics demonstrate success').
In fact, it is amazing how far the technology has moved on. My first defibrillator was powered by steam. Well, almost. Not true, of course, but you get the picture. Now the 'box', as the medical men like to call it, can do all manner of things. It can record and retain all unusual cardiac activity, which it can download to an external computer. It can pace your heart if it goes too slow (bradycardia). It can deliver 'pacing therapy' if you suffer a tachycardia (this involves the device introducing steadily increasing numbers of beats into the heart until the alien beat has caught up with the arrhythmia; it then catches the arrhythmia and is decelerated, so bringing the heart out of the tachycardia). And so on. I heard last we that we can now check the battery charge down the telephone. Of course, the medical bods all love it. Personally, I will do anything that keeps me out of hospital. Apart from that, I'm not bothered. (They would burn me at the stake at St George's for such treason).
If you are currently struggling with an implanted defibrillator, I hope that you will take some comfort from my experience. I have had some horrors but I am living evidence that learning to live with a defibrillator is not only possible but not that difficult.
I end this article with the same comment that I end all such articles - If I can find a full, satisfying and confident life after cardiac meltdown then so can anyone.
